Rates

Telephone/E-mail consultations: E-mail consultations
require that a signed informed consent of our e-mail
policy be on file. Phone consultations lasting more than
15 minutes incur a greater charge. E-mail consultations
that require multiple communications incur additional
charges. The fee for telephone and e-mail is NOT
charged in the following cases: when you require
clarification of on-going therapy and when the doctor has
asked you to call. If there is any question about this
service, please ask in your communication. The
physician will try to respond to communications within 24
hours. If the physician has not responded within 24
hours, please call or e-mail the physician again. Please

Rates

New patients are seen in a first office call.

Rate for First Office Call: $1

Rate for Return Office Call: Variable $85 typical.

Rate for Phone/E-mail Consultation: $30 minimum

Phone and e-mail consultations are only provided for

established patients, and only under special

circumstances determined by the physician.

Payment Policies

At this time we do NOT accept insurance.

Full payment for all charges is required at the time of service.

Form of Payment: We accept payment by cash or check.
Checks denied for lack of funds will incur a $35.00 fee.

Cancellation charge: We require 24 hours notice for canceled
or rescheduled visits. If not canceled more than 24 hours prior
to the scheduled appointment, there is a $35 cancellation fee.

Purchase of dispensary products: Credit on account is given
for unopened items in perfect condition if returned within 30
days. Injectibles, injection supplies and products packaged in
the clinic cannot be returned.
