Patient Instructions for Return Office Visit
Prior to your first visit:
1. Please download and complete the following forms:
The day of your appointment:
1. If you have not completed all the above forms or reviewed the policies, please arrive early to do so before meeting with the
Doctor.
At your appointment with the doctor:
1. Your goals for the day’s visit and your health concerns are addressed.
2. If the appointment is for an adjunctive treatment modality, then the treatment is performed.
3. If the appointment is for follow-up care, then the following are performed:
review of symptoms, physical examination and test ordering is performed.
Treatment during the return visit:
1. Adjunctive Treatment may be performed.
2. Doctor and patient may review and update the treatment plan to support patient compliance and wellbeing.
3. Nutritional supplements and botanical remedies may be prescribed for general health support and prevention.
4. Nutrients, botanicals, glandulars, and homeopathic remedies directed toward a specific condition or body system may be
prescribed, as needed.
Future treatment:
1. Appointments for additional treatment modalities and follow-up care are scheduled according to the updated treatment plan
devised by doctor and patient.

- Perform an abbreviated physical examination.
